- Printable pdfs - Forms you may print and complete by hand or fill in electronically and save and submit to Enrollment Services.
- Online - Forms you may complete and submit online using any electronic or mobile device.
How to submit your printed paperwork or saved pdfs to Enrollment Services
- Upload your printed and scanned paperwork into the Virtual Drop Box.
- Email your completed paperwork to studentrecords@cptc.edu.
- Drop off the printed and completed paperwork at the CPTC Drop Box located in Building 17 on the Lakewood Campus.
- Mail completed printed paperwork to Enrollment Services, 4500 Steilacoom Blvd. SW, Lakewood WA 98499.
