CASE MANAGEMENT

This course introduces students to the fundamentals of case management practice. Students will review different models of case management and learn about common case management functions such as outreach, engagement, assessment, planning, accessing resources, coordination, and advocacy.
Learning Outcomes:- Demonstrate knowledge of fundamental case management skills.
- Describe the different models of case management presented during the course.
- Correctly identify case management functions such as outreach, engagement, assessment, planning, accessing resources, coordination and advocacy and their practical applications to the service population.
- Demonstrate the ability to produce a case management plan for a human services client.
